Job Description

Social Worker Care Management | Full-time, 80 hours bi-weekly Monday-Friday, 8am-4:30pm/12p-8pm Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge. Key Responsibilities:

Qualifications:
Education:
Bachelor's degree in Social Work required Master's degree preferred Bachelor's degree in Social Work required Master's degree preferred
Experience:
Minimum two years in acute care, SNF, physician offices, or community health settings Minimum two years in acute care, SNF, physician offices, or community health settings
Licensure/Certification:
LBSW license in Maryland required Must obtain care/case management certification within five years of hire LBSW license in Maryland required Must obtain care/case management certification within five years of hire
Skills & Abilities:
