Job Description
Hospice and Palliative Care Social Worker Wings of Hope is seeking a Clinical Social Worker to be responsible for developing, implementing and managing our hospice and palliative care patients. This includes but is not limited to identifying patient and family needs, connecting them with community resources, including grief support, and supporting the family systems. As part of the interdisciplinary team, this individual will educate and support hospice and palliative care staff, client and family/caregivers. Wings of Hope Hospice provides care to patients wherever they live, in private homes, assisted living facilities, and nursing homes. This position is full time, salaried with benefits, including health insurance, flex savings, retirement savings plan, life insurance and generous paid time off. Must be a graduate of a BSW in Social Work through a school that is accredited by the Council on Social Work Education, experience and have or be eligible for a current Michigan Social Work license. MSW strongly preferred. Wings of Hope Hospice & Palliative Care is a small non-profit jewel serving two counties and surrounding communities in rural southwest Michigan. We are a legacy hospice, formed in 1983 by a group of volunteers. Our community generously supports us as our work has gently touched the lives of many of our families, friends, neighbors and co-workers.
