• . We're looking for a dedicated professional who is passionate about serving the geriatric population, experienced in social services delivery, and skilled in leading and mentoring a team. In this leadership role, you'll help shape a supportive, person-centered environment for our residents and families, while working alongside a committed interdisciplinary team.

• Shift:

• Monday-Friday 8:30am-5pm

• What You'll Do:

• + Lead and manage all aspects of the Social Services Department.

+ Provide support for residents and families including assessments, discharge planning, and care coordination. + Supervise and mentor staff, promote professional growth, and ensure regulatory compliance. + Serve as a key liaison between residents, families, staff, and external care partners. + Uphold excellence in service and communication, helping create a community of dignity and respect.

• Requirements:

• + Bachelor's degree in Social Work or a related human service field + Licensed Social Worker (LBSW or LMSW) in Michigan + At least 1 year of supervised social work experience in healthcare + 1-2 years in a supervisory role, preferably in long-term or geriatric care + Knowledge of MDS, discharge planning, and state regulations If you're ready to lead with heart and help enrich the lives of our residents, we'd love to meet you.
