• to join our Social Services Department and play a vital role in enhancing the lives of our residents and their families.
• About the Role
• As a Social Worker at Glacier Hills, you will be an advocate, resource, and trusted partner in care.

Your responsibilities will include:

+ Participating in discharge planning, resident assessments, and the development and implementation of individualized social care plans + Conducting cognitive and mood assessments and coordinating appropriate follow-up care and treatment + Completing timely and accurate documentation, including medical forms, reports, evaluations, and charting + Referring residents and families to community resources and appropriate social service agencies + Educating residents and families on Medicare, Medicaid, and other financial assistance programs

• About You
• You are compassionate, organized, and committed to resident-centered care.

Qualified candidates will have:

+ Active Social Worker licensure in the state of Michigan + A Bachelor's or Master's degree in Social Work or a related human services field (such as Sociology, Psychology, Special Education, or Rehabilitation Counseling) + One year of supervised social work experience in a healthcare setting (preferred) + Experience working in long-term care or with the geriatric population (preferred)
