Active Michigan LMSW Hospice Licensed Social Worker MSW Job Summary:
We are seeking a compassionate and skilled Hospice Licensed Social Worker MSW to join our team at Affinity Hospice Management LLC in Detroit, Michigan. As a core member of our Interdisciplinary Group, you will provide psychosocial support to our patients and their families, assessing their needs and developing individualized plans of care. Responsibilities:
Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse, and plan interventions based on evaluation findings Educate patients and families on, and assist in, preparation of advanced directives Participate in the development of individualized plans of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings Communicate with the physician regarding the patient's needs and report changes in the patient's condition Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs Maintain collaborative relationships with organization personnel to support patient care
Requirements:
Master's degree in Social Work (MSW) from a school accredited by the Council on Social Work Education Minimum of one (1) year experience in healthcare, hospice experience preferred Demonstrates good verbal and written communication, and organization skills Possesses and maintains CPR certification (may not be internet-based unless the demonstration of skills was hands-on and observed by a certified trainer) Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
