• LMSW Social Worker

POSITION DUTIES

• Delivers direct support to pediatric and adolescent students and their families in a home
• care setting, addressing personal, social, and environmental challenges that affect well
• being and school success.

They collaborate closely with students, families, and school staff to develop and implement effective care plans, provide referrals to community resources, coordinate services to ensure quality and efficient use of supports, and communicate clearly to promote satisfaction and positive outcomes throughout all phases of the social work process.

MINIMUM REQUIRED QUALIFICATIONS

• MI LMSW or LLMSW Masters of Social Work degree from an accredited program Valid Michigan driver's license and a dependable vehicle insured as required by the State of Michigan 5+ years case management experience with acute inpatient case management experience during the past 2+ years Discharge planning experience

PREFERRED QUALIFICATIONS

• Knowledge about the care of the pediatric and adolescent patient; Previous home care experience

LENGTH OF ASSIGNMENT

• 1+ year

SHIFT / HOURS PER WEEK

• 8 hour and 10 hour shifts available, no holidays, weekend rotation required. Later shift would be heavily supporting the ED. 8's: 8am-4:30pm or 3pm-11:30pm (ED). 10's: 3pm-1:30am (ED) SYSTEMS
