BSW HOSPICE SOCIAL WORKER-FULL TIME

Job Overview Summary :

Provides social work services to clients in their home according to the plan of care. Provides services to resolve social or emotional problems that are or expected to be an impediment to the effective treatment of the client's medical condition or his or her rate of recovery. Provides and coordinates skilled home care services that reflect the mission, values, philosophy, policies/procedures and standards of care that A&D Health Care Professionals is committed to uphold. Reports to the Clinical Supervisor/Branch Manager or designee.

Qualifications:

Currently licensed as a certified social worker eligible to practice in the State of Michigan. Minimum of one-year social work experience in a health care setting, home care/hospice experience preferred. Bachelor's degree from a school of social work accredited by the Council on Social Work Education Knowledge of Medicare-certified home health/hospice care preferred. Knowledge of community resources. Meets health requirements demanded of all clinical/field staff who provide service through the agency.

Skills and Abilities:

Must be able to work as scheduled and report to work on time. Must be able to operate a motor vehicle and to travel within A&D service area. Must possess excellent interpersonal and telephone skills. Possess excellent customer relation skills. Able to communicate effectively, both verbally and in writing. Must be able to complete multi-tasks, meet multi-deadlines and work in a stressful environment. Has effective time management skills and excellent organizational skills. Able to report for work on a regularly scheduled basis. Has reliable personal transportation.
