How you'll make an impact in this role Investigate, assesse and plan interventions to help participants cope with social, emotional, economic and environmental problems. Interview participants, assess priorities and document case activity. Consult with team managers, statutory and voluntary agencies and participants' caregivers to ensure compliance with participant's treatment plan. Assist participants/caregivers with obtaining community assistance by referral to proper resources. Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the participant with their needs. Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs. What minimum requirements you'll need Licensure / Certification /

Registration:

Preferred Credential(s): Drivers License. Masters Level Social Worker.

Education:

Master's degree in Social Work required.

Work Experience:

3 years of experience preferred.
