POSITION SUMMARY
The Social Worker is a multidisciplinary team member reporting to the Social Work Director and is responsible for ensuring that the medically related emotional and social needs of the patients are met and maintained on an individual basis in accordance with policies and procedures, and current TJC, federal, state, and local standards, guidelines, and regulations.
QUALIFICATIONS
Currently a Licensed or Limited License Master Social Worker in the State of Michigan Preferably three years (3) experience in a clinical setting or as a licensed counselor. Demonstrated competency in diagnostic developmental assessments. Demonstrated knowledge of health care delivery, proven ability with handling documentation in an organized and efficient manner and working well within an interdisciplinary team. Demonstrated ability to maintain and manage a caseload, complete records in a timely manner, to provide a variety of services, and to work harmoniously with professional and clerical personnel. Demonstrated knowledge of abnormal psychology, developmental disabilities, personality theory, community systems theory and practice, psychological testing, and counseling theory and practice. Demonstrated skill in written and oral communication. AGE(s)
OF PATIENTS SERVED AND AGE-SPECIFIC TECHNOLOGY
Adult Population:
Age 18 and up Sufficient knowledge of Growth and Development to interact with specific age groups. Will have sufficient knowledge in adapting interventions and communication styles to meet the developmental needs of all clients.
DUTIES AND RESPONSIBILITIES
The duties set forth below describe the general nature and level of work being performed by the social worker. They are not intended to be construed as an exhaustive list of all responsibilities, duties, and skills required of the social worker. Performs social service admissions functions including the development of comprehensive social history and psychosocial assessments. Participates in the development of individualized patient care plans involving the patients and their families in care planning. Attends scheduled care conferences and identifies and interprets psychosocial needs of patients for inclusion in the care plan, addressing issues of the care schedule, treatment, and environment that express and reinforce individuality and identity. Completes mandatory documentation within timelines established by applicable regulations. Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs. Plans discharges/transfers to ensure appropriateness and continuity of care. Emphasizes the importance of safety, patient rights, and person-centered planning in all aspects of care. Utilizes a problem-solving approach to resolving problems with other clinical services and the Medical Staff. Promotes harmonious relationships and positive attitudes with other departments and Medical Staff. Participates in the planning of multi-disciplinary projects for the Center. Represents the Center in community activities and educational programs. Participates in in-house educational programs. Maintains licensure standard of CEUs as required by the MDCH licensing board. Performs other duties in a willing manner or as requested by the Social Work Director and other professional and non-professional staff, as required.
