Job Description

JOB RESPONSIBILITIES
• Conduct outreach activities to identify and area students in need of health services. Provide health education and information to individuals and groups on topics such as chronic disease management, nutrition, mental health and preventive care. Assist patients in navigating the healthcare system, including helping them access medical, dental, social, and community services. Develop and maintain relationships with community organizations and resources to facilitate referrals and support for patients. Collect and report data on outreach and patient interactions to support program evaluation and improvement. Participate in community events and health fairs to promote health education and services. Obtain and maintain CHW certification and stay current with best practices, continuing education and guidelines in community health work. Collaborate with healthcare providers and other team members to coordinate patient care and support. Advocate for patients and help them overcome barriers to accessing healthcare and other services. Medical insurance services and enrollment Assist in administering risk assessment tools Coordinate and facilitate Community Advisory meetings Assist with data collection and entry for grant reporting purposes
JOB REQUIREMENTS
All of the following functions, qualifications, knowledge, skills, abilities (KSA's), and duties are essential. An employee in this position, upon appointment, should have the equivalent of the following: Valid driver's license. Travel between locations with the use of a personal vehicle. Excellent communication and interpersonal skills, with the ability to interact effectively and respectfully with diverse socio-economic levels. Strong organizational skills and the ability to manage multiple tasks and priorities. Ability to work independently and as part of a team. Knowledge of community resources and the ability to connect patients with appropriate services. General knowledge of Microsoft office Experience in health education and outreach preferred. EMR experience preferred Ability to learn quickly and adapt daily practices to changing guidelines and circumstances.
EDUCATIONAL REQUIREMENTS
Certification as a Community Health Worker or willingness to obtain. High school diploma or equivalent
INTRODUCTION
The Community Health Worker (CHW) is responsible for providing health education, support, and outreach services to the community. This role involves working closely with patients in area schools to improve their health outcomes by connecting them with necessary health services and resources. This position is funded in whole or in part through grant funding. Continued employment is contingent upon the availability of grant funds and ongoing organizational need. In the event grant funding is reduced or discontinued, the position may be subject to modification or elimination.
