Social Worker What You Must Have:
Bachelors or Master's degree in social work from a State accredited program Current State license required in accordance with State regulations Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred Full Time Employee Benefits Competitive Pay Medical, Dental & Vision insurance Paid Time Off Paid holidays 401k with up to 4% employer matching Tuition reimbursement Company car for qualifying individuals Mileage reimbursement
What You Will Do:
Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals. Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues. Facilitate community resources based on patient/family needs. Accept clinical assignments that are consistent with education and competence to meet the needs of the patients Perform on-call responsibilities and provide on-call service to patients/families as assigned Timely submission of all documentation Performance of other duties as required Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures It is this Agency's policy to provide equal employment opportunities without regard to age, race, color, religion, military status, gender preference, sex, marital status, national origin or disability.
