Master Social Worker (LMSW or LLMSW) The P rogram of A ll-Inclusive C are for the E lderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and - through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides - coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. We are seeking a dedicated Social Worker (LMSW/LLMSW) to join our interdisciplinary care team in providing comprehensive services for older adults. In this role, you will coordinate social service programs, conduct in-home assessments, manage community resources, and collaborate with participants' families to enhance their quality of life. The ideal candidate will hold a master's degree in social work, be fully licensed in Michigan, and possess at least one year of experience working with the frail elderly or long-term care population.
Key Responsibilities:
Conduct in-home comprehensive assessments and develop care plans. Manage community resources effectively. Collaborate with participants' families and facilitate meetings. Act as an expert in Advance Care Planning. Provide education to alleviate caregiver burden.
What We Require:
Education & Licensing:
Must possess a Master's Degree in Social Work (MSW) and be licensed in the State of Michigan.
Experience:
Minimum of one year of experience working with frail elderly or long-term care populations.
Skills:
Detail-oriented with strong problem-solving abilities. Excellent written and verbal communication skills. Ability to maintain discretion and confidentiality. Strong organizational skills and proficiency in MS Office Suite (Word, Outlook, PowerPoint, Excel).
Certifications:
Current CPR certification by AHA, or willingness to obtain it through Senior Care Partners P.A.C.E.
