SPECIFIC DUTIES
:
  • Coordinates and provides access to individual and family supports, including referral to community social supports.
  • Meets regularly with the care team to plan care and discuss cases and exchanges appropriate information with team members in an informal manner as part of the daily routine of the clinic.
  • Identifies community resources (i.e. social services, workshops, etc.) for patient to utilize to maximize wellness and recovery capital.
  • Conducts referral tracking.
  • Coordinates and provides access to chronic disease management including self- management support.
  • Implements wellness and Prevention initiatives.
  • Facilitates health education groups.
  • Provides education on health conditions and strategies to implement care plan goals including both clinical and non-clinical needs.
  • Conduct a risk assessment/needs assessment with the patient.
  • Obtain all appropriate releases and required documentation.
  • Develop treatment goals to be added to the care plan with the patient.
  • Handles all patient information in a confidential manner
  • Manage caseload.
  • Maintain documentation effectively.
  • Complete case reviews.
  • Performs other related duties as assigned.
SKILLS NEEDED
[1]:
  • Considerable knowledge of and ability to apply empathy, crisis intervention and substance abuse counseling techniques.
  • Basic knowledge of DSM-V and the ASAM placement criteria.
  • Considerable knowledge of community service organizations and the type of assistance they provide, including treatment resources as needed by patients.
  • Knowledge of current substance abuse treatment and rehabilitation methods.
  • Knowledge of, or the ability to learn, the operations and legal requirements of a substance abuse program.
  • Considerable ability to communicate effectively both in oral and written form.
