Under the supervision of Public Health Director, functions as an advocate and navigator for families (start of pregnancy through child enrolling in school age) encountering the child welfare system. Provide education and support to families through home visiting services using the Family Spirit Model. Families served will be identified through tribally established criteria and who are at risk of experiencing child protective services intervention. Ensures coordination, and communication across services, agencies and community partners, both tribal and non-tribal. Must be able to work independently and as part of a team. Perform tasks with dedication to quality service and adherence to the core values of the team. Provide leadership through respect, credibility, problem solving, reliance on accurate information and resources, and focus on mission. Establish and maintain effective and non-judgmental working relationships with families and providers within local and tribal service systems. Highest standards of confidentiality must be maintained according to both federal and state guidelines regarding all contacts. All Public Health staff must adhere to mandated reporting laws. This is a grant funded position.Essential Duties and Responsibilities:
Undergo extensive training to ensure and maintain core competencies and knowledge reflecting Indian Child Welfare Act (ICWA) and Michigan Indian Family Preservation Act (MIFPA) child custody proceedings and placements Participate in effort to inventory and map local resources and services for families experiencing or at risk of removal Serve as the lead point person, Tribal Liaison/Family Spirit Home Visitor responsible for ensuring access to high quality services and coordination between local, tribal and state agencies Meet with families one-on-one and/or along with other local, tribal and state agencies; Serve as a liaison between and among local, tribal and state agencies, including tribal court, well-child, home visiting, child welfare, substance abuse treatment, recovery support, MAT, social services, legal services, educational services, child care, employment support, housing, etc. Monitor services for all identified families, providing trouble shooting, navigation and support as needed, such as transportation, assistance with referrals, scheduling appointments, providing education and support around proper medication use, completing support plans for every family and ensuring cross-provider/cross-services communication Perform data collection and documentation necessary for compliance, providing requested information for grant reporting to the Inter-tribal Council as well as to appropriate Tribal department. Provide excellent customer service for all internal and external customers of the operations at all times. Provide solutions for customer concerns and continually focus on customer service as our top priority.Contacts/Purpose of Contacts:
Direct contact with Nimkee medical staff for referrals and services. Contact with Tribal Community Members, State and Local Law Enforcement Agencies, Tribal Court, Tribal child placement agencies, health and human service agencies and service providers Tribal Departments for coordination of care and event planning. Contact with outside providers for coordination of care. Direct contact with patients All levels of personnel in order to communicate operational decisions. Frequent contact with administrative staff, the Tribal community, and members of the general public as individuals or groups. Contact with local, county, state, and private social service agencies.Minimum Qualifications:
Bachelor's degree in Public Health, Health and Human Services, Social Services, Behavioral Health, or related field. Minimum 3 years of relevant experience in field of child protective services, behavioral health, criminal justice, community health, prenatal health care management, or social services. Program planning and coordination of program experience. Must be able to effectively communicate and have good computer skills. Must be able to pass background check to meet the employment eligibility requirements as they pertain to the position.Desired Qualifications:
Credential or certification in substance abuse, behavioral health, social work, criminal justice or related fieldWork Environment:
Normal office environment and home setting. Exposure to bodily fluids such as blood and saliva and/or the potential for needle sticks and/or skin puncture. Employee may be exposed to unpleasant odors, radiation, blood-borne pathogens, hazardous material and infectious disease. Extended hours and irregular shifts may be required.
