Join our collaborative care team at Carease Health - MI, where you'll have the opportunity to make a direct impact on the lives of older adults. As a Social Worker/Care Companion, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. If you're passionate about helping others and want to bring comfort and connection to those who need it most, we encourage you to apply.
Key Responsibilities:
Visit designated facilities in accordance with the schedule set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric
NP Qualifications:
Associates degree in Social Work, Psychology, Counseling, or another mental health-related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills 1 year of experience Why Join Carease Health?
