We are seeking a compassionate Social Worker (LMSW or LBSW) to join our interdisciplinary care team and help make a meaningful difference in the lives of our residents and their families.

Full-Time Schedule:
40 hours per week
Pay:
Competitive, based on e education, licensure, and relevant experience What You'll Do Complete admission and psychosocial assessments. Participate in interdisciplinary care planning and care conferences. Provide resident and family counseling and support. Coordinate discharge planning and community resources. Facilitate advance directive discussions and advocate for resident rights. Address quality-of-life concerns and support residents through adjustment, grief, behavioral, and family-related issues. Ensure residents' social service needs are identified, addressed, and appropriately documented. Maintain accurate documentation and compliance with HIPAA and regulatory requirements. What We're Looking For Bachelor's in Social Work. Current Michigan Licensed Bachelor Social Worker (LBSW) license required. Experience in rehabilitation, long-term care, skilled nursing, or healthcare preferred. Strong communication, organizational, and interpersonal skills.
