Full Job Description Position Summary:
The Staff Social Worker provides medically related social services to residents, ensuring that psychosocial needs are met and that residents attain or maintain their highest practicable well-being. This role advocates for residents' rights, supports discharge planning, and collaborates with interdisciplinary teams.
Key Responsibilities:
Conduct psychosocial assessments and update care plans based on resident needs. Provide counseling and supportive services to residents and families. Assist with admission, discharge, and transfer processes, ensuring appropriate communication with physicians and families. Facilitate discharge planning, including coordination with community resources and services. Advocate for resident rights and provide education to staff on psychosocial issues. Document social services interventions and progress in resident records. Participate in interdisciplinary care conferences and survey preparation. Skills & Competencies Knowledge of long-term care social work practices and regulations. Strong communication and counseling skills. Ability to work effectively with residents, families, and interdisciplinary staff. Organizational and problem-solving abilities.
Required:
Bachelor's degree in Social Work or related field preferred.
Preferred:
Experience in long-term care or healthcare social services. Physical & Work Requirements Ability to move freely throughout the facility and interact with residents and families. Occasional lifting of up to 40 lbs and pushing wheelchairs as needed.
