Job Description
Amara Hospice Care Social Workers perform the social services section of the assessment process; including but not limited to, completing the psych-social assessment, educating the patient and family about the Hospice benefit and gathering financial information. Develops a plan of care with the interdisciplinary team, the patient and the family to deal with personal, financial and environmental difficulties experienced by the patient. Assists the Interdisciplinary team to understand the significance of social, emotional and financial factors related to the patient's care. Assesses and reassesses social, emotional and financial factors in order to help the patient and family cope with problems related to the patient's lie limiting illness. Identifies and utilizes community and family resources to assist with the patients plan of care. Develops, prepares and maintains clinical documentation with accuracy, timelines and according to prescribed policies. Must be an LSW in the state of MI, One year hospice experience Willing to travel Be familiar with the territory Full-Time Position. Hours 8:30-4:30 Monday thru Friday. Must work independently but also work well within an Interdisciplinary team. The Amara Hospice Care goal is to provide end-of-life care and supportive services for patients and their families, and to enhance their quality of life. Our hospice team is dedicated to promoting a state of serenity of mind, body and soul for our patients and their families by collaborating with families to provide quality and compassionate hospice care. The Social Worker is responsible for assessing the spiritual and religious needs of patients and their families, arrange for the provision of spiritual and religious support in accordance with their wishes, and supporting families in the bereavement process. Must have a minimum of one Clinical Pastoral Education (CPE) unit. Prior Hospice experience is preferred. This position requires travel and covers Saginaw county and surrounding areas. Minimum of one year supervised experience in a medical, educational or religious agency/institution. Ability to provide an inter-faith approach to spiritual care demonstrating a high level of cultural and theological competence in working with diverse faith and belief systems. Demonstrate excellent oral and written communication skills, presentation skills, and teaching skills.

Computer skills:
Word, email, patient database charting, internet, voicemail. Must have a reliable vehicle, valid driver's license, and the minimum state required liability auto insurance.
