Position Summary The Hospice Social Worker is an integral member of the interdisciplinary hospice team, providing emotional, psychosocial, spiritual, and practical support to patients and their families. This role involves assessing patient and family needs, coordinating community resources, facilitating care planning, and supporting families through the end-of-life journey and bereavement process. This is a full-time position covering Metro Detroit and surrounding areas.
Schedule:
Monday through Friday, 8:30 AM - 4:30 PM Essential Responsibilities Complete comprehensive psychosocial assessments for hospice patients and families. Educate patients and families regarding hospice benefits and available services. Gather and evaluate financial, social, emotional, and environmental information that may impact care. Collaborate with patients, families, and the interdisciplinary team to develop individualized plans of care. Assist the care team in understanding the social, emotional, cultural, spiritual, and financial factors affecting patient care. Reassess patient and family needs throughout the hospice journey and adjust interventions as needed. Identify and coordinate community, family, and support resources to enhance quality of life. Provide emotional support, counseling, and crisis intervention for patients and families. Assess spiritual and religious needs and coordinate support services in accordance with patient and family wishes. Support families throughout the bereavement process. Maintain accurate, timely, and compliant clinical documentation in accordance with agency policies and regulatory requirements. Participate actively as a member of the interdisciplinary hospice team. Qualifications Required Licensed Social Worker (LSW) in the State of Michigan. Minimum of one year of supervised experience in a medical, educational, social service, or related setting. Excellent oral, written, presentation, and teaching skills. Strong organizational skills and ability to work independently while collaborating effectively within a team environment. Proficiency with Microsoft Word, email, electronic medical records, internet applications, and voicemail systems. Reliable transportation, a valid driver's license, and state-required automobile liability insurance. Willingness to travel throughout the assigned service area. Familiarity with Saginaw County and surrounding communities. Commitment to Amara Hospice Care's mission, values, and philosophy of compassionate end-of-life care. Preferred Previous hospice or palliative care experience. Experience working with diverse populations and providing culturally sensitive care. Knowledge of community resources and support services.
