In this vital role, you will provide comprehensive social work services to patients in post-acute, inpatient, outpatient, and long-term care settings. Your expertise will support patients' emotional well-being, facilitate discharge planning, and coordinate community resources to ensure holistic care. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring specialized nursing care, including those with complex medical and behavioral health needs. Your energetic approach and commitment to excellence will help foster a supportive environment for patients and their families. Duties Conduct thorough patient assessments, including psychosocial evaluations and diagnostic evaluations, to inform individualized care plans. Provide clinical counseling, individual and group therapy sessions, and behavioral health interventions tailored to diverse patient populations such as seniors, children, or individuals with disabilities. Coordinate discharge planning by collaborating with healthcare teams, families, and community resources to ensure seamless transitions from hospital or nursing home settings. Offer crisis intervention and motivational interviewing to support patients facing mental health challenges or end-of-life decisions. Maintain accurate medical documentation adhering to HIPAA regulations and social work practice standards; utilize EMR systems for efficient record keeping. Engage in case management activities including data collection, treatment planning, and follow-up to optimize patient outcomes. Support specialized care initiatives such as hospice & palliative medicine, Alzheimer's care, PTSD care, autism spectrum disorder interventions, and working with individuals with developmental disabilities. Requirements Minimum of Bachelor's Degree in Social Work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology, Master's Degree preferred. Proven experience in hospital settings, nursing homes, assisted living facilities, or outpatient clinics with familiarity in inpatient experience and post-acute care. Current Social Work license in the state preferred Knowledge of regulatory standards and compliance requirements. Working knowledge of personal computer and software applications used in job functions, (word processing, graphics, databases, spreadsheets, etc.) Strong organizational and analytical skills; oral and written communication skills. Working knowledge and ability to apply professional standards of social work practice in job situations. Current knowledge of state and federal laws and regulations that apply to practice of social work in long-term care.
