What You'll Do:
Conduct psychosocial assessments and document patient/family needs Offer therapeutic support and grief counseling Assist in navigating care planning and resource access Act as an advocate and educator for patients and caregivers Be present in life's most vulnerable moments—with empathy and professionalism
Qualifications:
Degree in Social Work (BSW or MSW) Hospice experience is required Exceptional communication and emotional intelligence Team-oriented with the ability to work independently Familiarity with the grief process and end-of-life frameworks
