We are seeking a Hospice Social Worker to provide psychosocial assessment, counseling, education, and care coordination for hospice patients and their families. This role is essential in supporting emotional well-being, care planning, and dignity at end of life while collaborating closely with an interdisciplinary care team.

Schedule:

Full-Time (8:00am- 4:30pm weekends on Call PRN)

Paul Wage:

$31.35-$39.45/hr. DOE What You'll Do Report to work as scheduled, prepared to perform all job duties Participate in daily team huddles and assess critical psychosocial needs requiring immediate attention Schedule and conduct daily patient and family visits Complete initial psychosocial and bereavement assessments within five days of hospice admission and communicate findings to the interdisciplinary team Assess environmental, psychosocial, and mental health factors including family dynamics, caregiver capacity, support systems, communication patterns, mental health risks, abuse, neglect, and safety concerns Provide counseling and develop individualized care plans based on psychosocial assessments Educate and support patients and families regarding financial, legal, healthcare concerns, and expectations related to the death and dying process Identify and respect cultural, spiritual, and belief-based traditions or rituals and communicate needs to the care team Assist patients and families with funeral and burial planning Participate in interdisciplinary team meetings and care conferences, providing routine updates on patient plans of care Complete all clinical documentation in the Electronic Medical Record (EMR) in compliance with documentation standards Review and educate patients and families on advance care directives to ensure wishes are documented Coordinate referrals and assist with practical, environmental, and community resource needs Collaborate with the care team to ensure patient safety, care planning, and support needs are met Ensure traveling contracts are in place when patients travel outside the service territory Implement safety and behavioral contracts as needed Ensure patients are registered with the local Medical Examiner as required Participate in quality improvement initiatives Perform other duties as assigned by leadership What We're Looking For Strong interpersonal, verbal, and written communication skills Excellent organizational, time management, and documentation skills Ability to work independently and collaboratively within an interdisciplinary team Commitment to patient dignity, safety, and compassionate end-of-life care Qualifications Master's degree in social work (MSW) Hospice experience preferred Must have and maintain licensure to practice as a Social Worker in the State of Minnesota in accordance with Minnesota Statute 418.114(b)(3) Valid Minnesota driver's license Reliable, fully insured vehicle meeting state and organizational requirement Strong spoken and written English skills required to perform job duties, communicate with team members, and understand work-related documents.
