Maintain a clean, safe, and comfortable environment while following all care plans, safety protocols, and infection control standards. Leads and directs Social Service Department. Coordinates resident admissions. Assist with prompt referral to an appropriate agency or government program if financial assistance is indicated; assist with re-certification papers to maintain status of government programs such as Medical Assistance, Social Security, and S.S.I.; notify Business Office. Completes assessments of residents as necessary for care planning, MDS, RAPS, PPS and other required documents. Serves as resident advocate
  • coordinates invitation of residents and families.
