Job Description

Job Summary We are seeking two reentry coordinators and two reentry support staff. Both will support individuals transitioning back into the community after involvement with the justice system. In this vital role, you will serve as a guiding resource, helping clients navigate complex social, legal, and personal challenges. Your energetic approach will empower clients to achieve stability, build positive relationships, and access essential services. This paid position offers an opportunity to make a meaningful impact by fostering successful reintegration and reducing recidivism through proactive case management and motivational support. Reentry Coordinator The Reentry Coordinator serves as the primary case manager for residents participating in Next Chapter Reentry Project's residential reentry program. This position is responsible for resident accountability, case planning, documentation, resource coordination, and program compliance. The Reentry Coordinator works directly with residents to help them achieve goals related to employment, housing stability, recovery, family reunification, education, and successful community reintegration. The Coordinator serves as the primary point of contact for assigned residents and collaborates with probation officers, treatment providers, employers, and community partners to ensure residents receive the support necessary to successfully complete the program.
Primary Focus:
Case management, resident accountability, documentation, resource coordination, and program outcomes. Reentry Support Staff The Reentry Support Staff position provides supplemental support to residents and assists the Reentry Coordinators in carrying out daily program operations. This position helps create a safe, structured, and supportive environment by assisting with resident transportation, property checks, resident engagement, programming support, intake preparation, facility oversight, and other operational needs. Reentry Support Staff serve as an additional layer of support and encouragement for residents, helping maintain program standards and reinforcing expectations established by the Reentry Coordinators and the leadership team.
Primary Focus:
Resident support, operational assistance, transportation, facility oversight, programming assistance, and reinforcing resident accountability. Key Difference Reentry Coordinator Leads resident case management. Develops and monitors resident plans. Conducts resident meetings. Maintains documentation. Coordinates with outside agencies. Addresses behavioral and compliance issues. Responsible for resident outcomes. Reentry Support Staff Assists residents with daily needs. Provides transportation and practical support. Helps facilitate programming. Conducts property and room checks as assigned. Assists with intakes and move-ins. Supports Coordinators in implementing resident plans. Reinforces expectations but does not serve as the primary case manager. In short, the Reentry Coordinator manages the resident's overall progress and case plan, while Reentry Support Staff help residents successfully navigate day-to-day challenges and support the implementation of those plans. Conduct comprehensive assessments to understand clients' needs, strengths, and barriers to successful reentry Develop personalized reentry plans that include employment, housing, mental health, and social services Utilize motivational interviewing techniques to inspire clients toward positive behavioral change and goal achievement Collaborate with law enforcement agencies, community organizations, and service providers to coordinate resources and support networks Manage crisis situations with professionalism and sensitivity, ensuring client safety and stability Document all interactions thoroughly through detailed report writing and case notes in compliance with organizational standards Provide ongoing case management by monitoring progress, adjusting plans as needed, and offering consistent encouragement Qualifications Proven experience in social work, case management, or related fields with a focus on justice-involved populations Strong knowledge of law enforcement procedures and community resources relevant to reentry efforts Skilled in crisis management and conflict resolution to handle sensitive situations effectively Proficient in interviewing techniques that foster trust and open communication Familiarity with report writing requirements for documentation and compliance purposes Excellent organizational skills with the ability to manage multiple cases simultaneously Motivated by a passion for helping individuals succeed in their reintegration journey Preferred qualifications for reentry coordinators include experience with motivational interviewing techniques, law enforcement collaboration, crisis intervention strategies, and social work practices.
