Job Description

Join a collaborative transplant team as a social worker supporting patients and families through every stage of the transplant journey. This ambulatory role focuses on heart and lung programs, with opportunities to contribute across additional transplant services. You will provide clinical assessments, interventions, and care planning while partnering with an interdisciplinary team in a mission-driven environment. Key Responsibilities Conduct psychosocial and diagnostic assessments to identify patient and family needs Develop and implement individualized treatment and transition plans Provide clinical interventions, including crisis intervention and psychotherapy Deliver education and support to patients and their families •This position is not eligible for visa sponsorship with the exception of the TN visa classification; Also, Mayo Clinic

extension program. To be considered for this position, you must have a Master of Social Work degree (MSW) . Licensure Licensure at the Licensed Graduate Social Work (LGSW), Licensed Independent Social Worker (LISW), provisional LGSW, or provisional LICSW level from the Minnesota Board of Social Work (MN BOSW) is required. •

personnel, following 4000 hours of clinically supervised practice are eligible to apply for licensure at the LICSW level, and must by 8000 hours. •A condition of employment at the
LGSW/LISW
level shall be the ability to practice clinically through a MN BOSW approved Clinical Supervision Plan leading to LICSW attainment within 8000 hours of employment post LGSW or LISW. Licensure to practice independently at the clinical level requires either a Licensed Clinical Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) license from the MN BOSW. With this level of license, you will need two years of supervised therapy experience in behavioral health and/or medical social work. Additional Qualifications Advanced knowledge of categories in mental illness, personality development, knowledge of the types of psychiatric treatment, and skills in conducting individual, group, and family therapy Knowledge in adjustment to medical diagnosis and chronic disease. Strong human relation skills to interact with patients, patient's family members, consultants, residents, hospital personnel, community agencies, and other resources Psychosocial diagnostic skills Application Requirements •All must be included for your application to be considered: CV/Resume Cover letter Internal applicants must attach their 3 most recent performance appraisals. Evaluation must be completed by an organization listed as a member of the National Association of Credential Evaluation Services (NACES) (www.naces.org) or the Association of International Credential Evaluators, Inc. (AICE) (www.aice-eval.org).
