Why Social Workers Choose Superior Hospice $65,000-$85,000 annually (Full-Time) or $32-$42/hour (Part-Time) Mileage Reimbursement Flexible scheduling with the opportunity to transition into a full-time position Practice in a relationship-focused hospice where quality always comes before volume Join an interdisciplinary team that values your voice and clinical expertise Leadership that understands the emotional demands of hospice social work Meaningful autonomy while remaining fully supported by experienced clinicians Opportunity to help build a growing hospice organization from the ground up Develop lasting relationships with patients, families, and caregivers A mission-driven culture built around compassion, collaboration, and dignity What You'll Be DoingGuide Families Through Life's Most Important Conversations Complete comprehensive psychosocial assessments and contribute to individualized care plans Help patients and families navigate goals of care, end-of-life decisions, advance directives, and available community resources Provide emotional support, crisis intervention, grief counseling, and anticipatory guidance Advocate for patient wishes while helping families make informed decisions Connect caregivers with practical resources that improve quality of life throughout the hospice journey Partner With An Exceptional Clinical Team Collaborate closely with Registered Nurses, Chaplains, Physicians, Hospice Aides, and other interdisciplinary team members Participate in care conferences and interdisciplinary planning Help ensure every patient receives compassionate, coordinated, person-centered care Build trusted relationships with referral partners and community providers when appropriate Provide Thoughtful, High-Quality Care Maintain timely and accurate documentation while preserving meaningful patient interactions Support patients wherever they call home throughout our service area Balance regulatory excellence with compassionate, individualized care Help create an experience where every family feels heard, supported, and respected What We're Looking ForRequired Qualifications BSW, MSW, or LICSW from an accredited Social Work program Previous experience in Hospice, Palliative Care, Oncology, or another end-of-life care setting Exceptional communication and relationship-building skills Strong emotional intelligence and the ability to remain calm during difficult conversations Ability to work independently while collaborating with an interdisciplinary team Valid driver's license and reliable transportation Who Thrives Here?
