Sign-On Bonus Hospice Social Worker Opportunity:

Hospice Social Worker Responsibilities:
Be a core member of the Hospice team and provide support to patients and families' emotional, social, and economic needs relating to the impact of a terminal illness. Plan the transition and transportation of patients from hospitals to homes or other facilities, arrange services, and collaborate with doctors, nurses, and insurers. Plan and lead interdisciplinary meetings, facilitate referrals for community resources, and review critical access admissions and monitor patients with regularly scheduled admission visits.
Hospice Social Worker Qualifications:
Must have an Arkansas LCSW or LMSW or be eligible to obtain one Must have or be eligible to obtain
BLS Certification Surrounding Location & Community:
