Job Description

Preferred Hospice is a well-established hospice company with 5 offices across the state of Missouri-Dexter, Farmington, St Louis, Columbia, Ozark. We are looking for an experienced Social Worker to work as part of our hospice interdisciplinary team serving the Dexter, MO area. The ideal candidate would have some medical care experience and live within a commutable distance to the office.
JOB DUTIES
The Hospice Social Worker visits wherever the patient resides-private home, skilled nursing facility, assisted living facility, hospital. The Hospice Social Worker completes a psychosocial assessment on each assigned patient in a timely manner, and documents in the electronic medical record. The Hospice Social Worker utilizes community resources for patients, such as in-home services, Meals on Wheels, etc. The Hospice Social Worker assists the patient, family and caregiver in coping with issues related to death and dying and participates in grief support as needed. The Hospice Social Worker provides information, completes hospice admission paperwork and educates patients, families and the community on all aspects of hospice services.
QUALIFICATIONS
The successful candidate will have graduated from a School of Social Work approved by the Council of Social Work Education with a Master's or Bachelor's Degree in Social Work. •Other related degrees/areas of study will NOT be considered as they are not allowed by state regulations•Work hours are primarily Monday through Friday, 8am to 5pm, with the understanding a crisis can occur at any time. Flexibility with scheduling and hours a must. Hourly rate, mileage reimbursement provided. Compensation is based on years of experience. Computer skills for charting/EMR, basic use of Word, Excel, Email; smart phone for mileage apps, etc. Willing to train the right person. A loving compassionate heart is a must.
