POSITION DUTIES

•Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.

MINIMUM REQUIRED QUALIFICATIONS

•MO LMSW; BLS; 1 year social work experience in an acute hospital setting; Acute hospital experience in specialty during the past year with no more than a 90-day break
