Job Duties:
Responsible for coordinating and consenting to care for County wards by working with staff members on financial and legal matters, with community health workers on medical and mental health issues, with state and federal agencies on entitlement questions, and with facilities to assure appropriate living arrangements or placement for wards. Provides information that potentially affects eligibility for entitlement programs; interacts with wards, facilities, care providers and other entities; reviews, advocates, consents to and monitors treatment plans and care, medication, and hospitalization; crisis assistance and annual visits with wards and is on-call some weekends and evenings
Minimum Qualifications:
Bachelor's Degree in a social behavioral science field. One year of prior experience working with adults who have chronic mental illness, drug, and alcohol dependent persons, physically, cognitively, or intellectually disabled adults, adults with behavior disorders and the frail and demented elderly. Must have experience with Microsoft Word and Outlook. Must have a valid Missouri Driver's License. Must submit to and pass a pre-employment background check and drug screen.
