We are professional, agile and our goal is to Provide quality, compassionate healthcare to those experiencing homelessness and poverty. We're looking for a dedicated individual with a commitment to excellence to join our Case Management team. The successful candidate will be highly proficient in assessments and referrals for those experiencing homelessness, possess experience in an administrative role, and be capable of managing multiple client interactions daily. We're looking for a team player who will be a strong support mechanism for both internal and external contacts. Essential Duties and Responsibilities /

Core Competencies:

Provide street outreach services to persons experiencing homelessness Attend CoC Housing Placement & Outreach team meetings to report on the status of CBB patients that are on the By Name Registry Coordinate direct financial assistance to patients Assist with Coordination of benefits Assist with obtaining vital documents Responsible for all data collection and grant reporting activity The position will be able to provide for personal transportation to meet job expectations
