Hospice BSW (Bachelor of Social Work)

Three Rivers Hospice located in West Plains is actively seeking a compassionate and dedicated individual with a BSW degree, from an accredited school for Social Work, to work with the Interdisciplinary Team. In this pivotal role, you will provide emotional and psychological support to our patients and their families during end-of-life care, ensuring they receive the highest quality of support in their final days.

Responsibilities :

conduct comprehensive assessments of patients' social, emotional, and environmental needs to develop personalized care plans. Provide counseling and support patients and their families, helping them navigate emotional challenges and facilitating open communication. Advocate for patients' needs and preferences ensuring they are respected and honored throughout their care journey. Coordinate community resources and support services to assist patients and their families in accessing necessary care and benefits.
