Medicaid Waiver Reports to: Case Management Director Summary/Objective This position is involved in coordinating the Case Management Program, which consists of planning, arranging for, and coordinating appropriate home and community-based services which shall be provided to clients in a timely and cost-effective manner. The incumbent assigned to this position maintains considerable contact with service providers, consumers of services, civic groups, and the public. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Works with a Registered Nurse case manager in the team approach to screen referred clients and determine eligibility. During on-site visits using prescribed skills, assesses the client, formulates nursing diagnoses and needs of the client, notes medical diagnoses, establishes a plan of care, implements the plan of care after eligibility has been established and approval received from appropriate sources, evaluates results of the plan by monitoring clients at least monthly. Provides technical assistance to service providers involved in the care of clients. Assists in resource development and coordination of services, which services to limit duplication, assists in client targeting, and maximizes the use of information support systems. Develops individualized care plans for clients based on the assessment of client needs and on resource availability. Attends meetings, training sessions, and conferences. Performs other requirements related to the successful operation of the case management program. Work Environment While performing the duties of this position, the employee may travel by automobile and be exposed to changing weather conditions. The employee works in varied residential environments based on an assigned caseload. Physical Demands An employee must meet the physical demands described here to perform the essential functions of this job successfully. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is regularly required to stand, walk, and sit; use hands to finger, handle or feel objects, tools, or controls; and reach with hands and arms. The employee frequently lifts and moves up to 10 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Position Type/Expected Hours of Work This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5 p.m. Travel Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected. Required Education and Experience Bachelor's degree in social work or related field with an active, clear license to practice Social Work in the State of Mississippi and two (2) years of full-time experience working in direct service to aged and disabled individuals. Candidates with less than two (2) years of experience will require at least 90 days of orientation under the supervision of an established case manager with two (2) years of waiver experience. Additional Eligibility Qualifications Thorough knowledge of the principles, procedures, and techniques of professional social work; knowledge of community resources available to individuals and families; knowledge and skill in interviewing and assessment techniques; ability to work in a team relationship; and knowledge and understanding of psychological, social, health and economic factors influencing the attitudes and behavior of individuals and families, especially as they relate to the elderly. Ability to establish and maintain effective working relationships with others. Excellent verbal and written communication skills. Basic knowledge of word processing programs, spreadsheets, email platforms, and other communication tools. Must have and maintain a valid Mississippi driver's license.
