Case Management:
Facilitating Care Coordination and Discharge Planning Oversee the transition of care from inpatient to outpatient settings, ensuring seamless coordination of services for optimal patient outcomes. Inpatient Utilization Management Manage and review inpatient services to ensure appropriate utilization of resources and adherence to established guidelines. Interdisciplinary Team Collaboration Actively participate in interdisciplinary team meetings to develop and implement effective discharge plans, fostering communication and collaboration across care teams.
RN Qualifications:
Current Montana RN license or a multi-state compact license with authorization to practice nursing within the state of Montana; required. BSN preferred. Minimum of two (2) years' RN experience in a similar position within a medical setting (e.g. physician office, clinic, or hospital) required. Minimum of one (1) year case management experience preferred. RQI Provider (BLS) within 7 days of hire/transfer or before independent patient care, whichever comes first, required.
Social Worker Qualifications:
Master's Degree or Bachelor's Degree in Social Work, or related Social Service Field required. Experience in a medical or hospital setting, or discharge planning, highly preferred. Must have prior social services experience, preferably in a healthcare setting. Current CPR per assigned area(s) may be required. Both RN and Social worker require: Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy. Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently. Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.
Job Specific Duties:
Identifies clinical and social needs of patient related to admission and/or discharge. Formulates a plan for transition in a safe and timely manner to the least restrictive environment that can meet each patient's needs. Collaborates with stakeholders to address social determinants of health (SDOH) and a thorough admission and/or discharge plan as applicable to assigned area(s). Identifies patients at risk for readmission and works with the interdisciplinary team across the continuum to formulate and implement a discharge plan to decrease the risk for readmission. Acts as a resource to providers, patients, caregivers and interdisciplinary team regarding community resources and reimbursement as applicable to assigned area(s). Exhibits knowledge and skills necessary to provide for the physical, psychological, and cultural needs appropriate to the age, ethnicity and special needs of the patient. Acknowledges and advocates for a patient's right to self-determination and informed consent. Consistently reflects ownership and professionalism towards Case Management. Identifies areas in need of improvement and serves an active role in developing solutions. Exhibits the Logan Health Core Values in all interactions. Responds to, and provides direction in, a changing workload in a positive and professional manner. Consistently promotes the use of the most appropriate setting and level of care to optimize patient outcomes and ensure efficient use of healthcare resources. Identifies and obtains necessary provider documentation and orders to ensure compliance with all applicable Logan Health, State, and Federal requirements as they relate to Utilization Review, Discharge Planning, and Care Coordination processes as applicable to assigned area(s). Communicates frequently with patient and family/support to promote the development of a patient-focused plan, and ensure that the patient and family/support members are informed and involved in decisions about care. Maintains regular and consistent attendance as scheduled by department leadership.

PRN, Variable 8-hour day shifts. Shifts may fall anytime between Monday
  • Sunday.
Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed. PRN positions (also referred to as Casual, As Needed, or Per Diem) at Logan Health offer flexibility and are designed to provide coverage during busy periods, employee absences, and vacations, making them an excellent opportunity to support the team as needed. PRN employees can participate in a matching 401(k) plan and have access to the Employee Assistance Program. Additionally, they may qualify for hourly differential pay for certain shifts or hours, offering extra compensation based on department-specific scheduling needs. Pay rates and scheduling practices may vary by department.
