Monday through Friday, 8:00am to 5:00pm Job Summary- The Resource Coordinator provides case management support to participants. It also provides administrative support for participant transitions, clinical appointments, and medical records. This position orders and tracks incontinence supplies for participants. Qualifications
Education/Experience :
Bachelor's degree in social work, human relations or human services, or high school degree with three years direct work experience with Medicaid and/or elderly populations. One year of experience with the frail or elderly preferred. Analytical thinking and problem solving ability Excellent interpersonal, oral and written communication skills Meet a standardized set of competencies for the position description established by the PACE organization and approved by CMS before working independentlyLicensure, Registry or Certification Required :
Valid NC driver's license and a reliable vehicle.Immunizations:
Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.Pay Range:
$17.85/Hourly- $23.
