Public Service Loan Forgiveness About Position:

The primary responsibility of the Social Worker is to provide social work care management services to the participants of the Piedmont Health SeniorCare program; provides services such as participant screening, assessments, care management, supportive listening and coaching and referrals.

Schedule:

Monday through Friday, 8:00am

Education/Experience :

This position requires a master's degree from an accredited school of social work, 2 years professional level experience in a health related area. One year of experience with the frail or elderly preferred. Individual therapy skills for geriatric participants Analytical thinking and problem solving abilities Ability to handle multiple complex priorities Excellent external and internal customer service Problem solving skills

Licensure, Registry or Certification Required :

Master of Social Work (MSW).

Immunizations:

Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.

Pay Range:

$59,050

• $79,358 (commensurate with years of experience)
