Work is performed under the moderate supervision of the Social Work Supervisor. Qualification Requirements To perform this job successfully, an individual must be able to perform each essential function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Position Responsibilities Performs comprehensive assessments to identify all aspects of the beneficiary. Develops person-centered service plans to determine all formal and informal services. Provides multidisciplinary treatment team meetings with all providers to monitor health and wellbeing, and continued appropriateness of services.
Completes monitoring requirements to include:
telephone, face to face and electronic exchanges. Facilitates referral and linkage activities with medical, behavioral, social and other human services agencies. Manages a caseload of disabled adults. Performs assigned functions as a Disaster Ready Team (DRT) member, if appointed or volunteer. Performs related work as required. Qualifications Knowledge of the population, disability, and culture of the beneficiary being served. Knowledge of formal and informal assessment practices, to establish a rapport with the beneficiary and family. Considerable knowledge in recognizing indicators of risk (health, safety, mental health and substance abuse.) Knowledge of values that underlie a person- centered approach to providing services. Knowledge of services and interventions appropriate for assessed needs for the development of the service plan. Knowledge of emergency and disaster safety planning. Considerable knowledge of community resources and current laws, regulations, and policies surrounding medical and behavioral healthcare. Knowledge of outcome monitoring, preventative intervention, community, beneficiary-advocacy and peer support groups. Ability to establish and maintain effective working relationships with administrative superiors, members and families of caseload, and with healthcare providers and community resources. Ability to train or instruct lower-level social workers, students, or interns. Ability to express ideas clearly and concisely and to plan and execute work. Education, Experience, and Special Requirements Bachelor's degree in a human services or equivalent field from an accredited college or university with two or more years of community experience (preferably case management) in the health or medical field directly related to home-care, long-term care, or personal care, and the completion of a NC Medicaid certified training program within 90 calendar days of employment.

Regular Onslow County employees have the ability to participate in a wide array of employee benefits including: Health Insurance Health Insurance is provided by BlueCross BlueShield of North Carolina and covers a wide variety of healthcare services. Dental Insurance Dental insurance is provided by Delta Dental to full-time employees and covers a percentage of basic to major dental procedures, including orthodontia for children and adults. Vision Insurance Vision insurance is provided by MetLife to full-time employees and covers a variety of materials (contacts and glasses frames/lenses) and an annual eye exam. North Carolina Local Governmental Employees' Retirement System Eligible employees are automatically enrolled in the retirement system and must contribute 6% of their gross salary each pay period. Once eligibility requirements are met, employees will be able to receive monthly payments for life based on their salary, age, and years of service credit. NC401k The County contributes 1% of employees' gross salary into the 401(k) account regardless of the employee's deferral amount. In addition, the County matches employee deferrals up to an additional 4%. Vacation Leave Regular full-time employees earn 13.5 days of vacation leave per year. With each 5-year increment of employment, vacation accrual increases. Sick Leave Regular full-time employees earn 12 days of sick leave per year. Paid Holidays County employees receive paid holidays annually. The Board of County Commissioners approves and may adjust the schedule as deemed necessary. Teladoc Mental Health Services The County provides Teladoc Mental Health Services to help employees and their families with personal, family and job-related problems such as depression, and substance abuse concerns. Supplemental Benefits County employees are also offered a variety of supplemental benefits such as Life Insurance, Flexible Spending Accounts, 457b Deferred Compensation Plan, and MetLife Supplemental Plans.
