Join our multidisciplinary care team as a Social Worker II , where you'll play a vital role in supporting patients and families through complex medical, emotional, and social challenges. This role offers the opportunity to make a meaningful impact by guiding patients across the continuum of care, advocating for their needs, and ensuring safe, effective discharge planning. Essential Functions Identifies patients who would benefit from Case Management interventions through initial screening and assessment of discharge needs Conducts comprehensive interviews with patients and families to gather relevant psychosocial, emotional, and environmental information Completes psychosocial assessments in accordance with departmental, regulatory, and professional standards Develops, implements, and evaluates individualized psychosocial treatment plans utilizing appropriate social work modalities, including crisis intervention, individual and family therapy, and grief and bereavement counseling Initiates and coordinates appropriate internal and external referrals based on patient-specific needs Advocates for patients to address unmet needs and remove barriers to care Collaborates with the multidisciplinary team to develop and implement effective discharge plans that promote continuity of care Coordinates and facilitates patient and family care conferences as needed Identifies barriers to efficient and effective patient care and seeks strategies to eliminate those barriers Maintains current knowledge of federal, state, and local regulations impacting social work practice Demonstrates awareness of payer and reimbursement practices and guides patients and families through post-discharge care options Documents all assessments, interventions, and care coordination activities in the Electronic Medical Record in accordance with departmental standards Education, Experience, Licensure & Certification Master's degree in Social Work from a school accredited by the Council on Social Work Education (CSWE) Clinical licensure (or provisional status) preferred Certification as an Accredited Case Manager (ACM-SW) preferred Social work experience in a healthcare setting strongly preferred Skills & Qualifications Demonstrated competence in providing age- and developmentally appropriate care Ability to establish positive working relationships and contribute to a collaborative team environment Proven ability to manage multiple priorities in a fast-paced setting with frequent interruptions Adaptability and resilience in a rapidly changing healthcare environment Strong communication, critical thinking, and advocacy skills
