Job Description
Duties and Tasks Provide supportive counseling and emotional support to residents and families Assist with mental health assessments, behavior management, and crisis intervention Complete psychosocial assessments, social histories, and required documentation Participate in interdisciplinary care planning and resident care conferences Support resident rights, advocacy, dignity, and individualized care preferences Assist with admissions, orientation, transfers, and discharge planning Coordinate community services, referrals, transportation, and outside resources Help residents access financial benefits and support programs including Medicaid, Medicare, Social Security, and VA benefits Collaborate with physicians, psychologists, nursing staff, and other care team members Maintain accurate documentation and ensure compliance with state and federal regulations Minimum Qualifications Bachelor's degree in Social Work Licensed Social Worker (or ability to obtain licensure if required by state guidelines) preferred Strong communication and interpersonal skills Ability to work compassionately with aging adults, veterans, and families Strong organizational and documentation skills Ability to work collaboratively within an interdisciplinary team Strong computer skills, including proficiency with electronic health records (EHR) and Microsoft Office applications Previous social work or healthcare experience preferred Knowledge of long-term care regulations and resident rights preferred Experience with crisis intervention, counseling, or behavioral health preferred About Team ND "Far and away the best prize life offers is the chance to work hard at work worth doing." - Theodore Roosevelt More than 7,500 talented, hard-working people across sixty-three agencies have come together as Team North Dakota.
