Job Description

The Bereavement Coordinator is responsible for planning, implementing, and overseeing bereavement services for hospice families, caregivers, staff, and the community. This role provides grief support, education, counseling, support groups, and memorial services while ensuring compliance with hospice regulations and best practices. The ideal candidate is a compassionate professional who is passionate about supporting individuals through loss, works collaboratively with an interdisciplinary team, and is committed to delivering exceptional care. Key Responsibilities Develop, implement, and evaluate the hospice bereavement program. Complete bereavement assessments and create individualized bereavement care plans. Provide grief counseling, support, and follow-up services to families for up to 13 months following a patient's death. Contact bereaved family members within two days of a patient's death. Facilitate support groups, educational programs, and memorial services. Provide bereavement education to hospice staff, families, facilities, and community members. Maintain accurate documentation and records in compliance with hospice regulations. Collaborate with the interdisciplinary team to support patients, families, and caregivers. Participate in quality improvement initiatives and program evaluations.
Qualifications Required:
Master's Degree in Social Work, Theology, Pastoral Ministry, or related field. Experience in grief counseling and bereavement support. Valid driver's license, reliable transportation, and automobile insurance. Strong communication, organizational, and interpersonal skills.
Preferred:
Licensed Social Worker, Chaplain, or related certification in good standing. Minimum of two years of hospice experience. Recent patient care experience. Experience facilitating support groups and community education programs.
Pay:
$25.00 - $30.00 per hour
