Job Description

Job Overview We are seeking a dedicated and compassionate Social Worker with a Master's degree to join our healthcare team. The ideal candidate will possess a strong background in clinical and community-based social work, with expertise in providing comprehensive patient-centered care across diverse settings. As a Social Worker-Master's Prepared, you will play a vital role in supporting individuals and families through assessment, counseling, case management, and coordination of services to promote optimal health and well-being. This position offers an opportunity to make a meaningful difference in patients' lives while working within a dynamic, multidisciplinary environment. Responsibilities Assist the physician and other members of the care management team in understanding significant social, economic, spiritual, and emotional factors related to the patient's health problems to establish a plan of care that fosters the personal worth, spiritual well-being, and human dignity of each patient. Develop effective, working relationships with patients, families and staff for further understanding of significant social and emotional needs underlying patient's health problem. Develop and maintains community contacts, utilizing available community resources to meet patients' individual needs. Assesses the social, spiritual, and emotional needs/factors to estimate the patient's and family's capacity and potential to cope with the problems of daily living. Participates in the development and review of the plan of care for assigned patients. Observe, records, and reports social, spiritual, and emotional changes and provide guidance and coping strategies to patients and families. Documents all services in the patients' clinical record. Manage crisis intervention situations effectively, offering immediate support and stabilization for patients experiencing acute distress or behavioral health crises. Document all clinical encounters accurately in compliance with HIPAA regulations and maintain detailed medical records using electronic medical record (EMR) systems. Requirements Knowledge of patient care standards and practices. Knowledge of the community resources available for patients and the requirements needed to access those resources. Knowledge of HIPAA privacy rules. Skill in communicating in a professional manner, both verbally and in writing. Skill in planning, organizing, and time management. Skill in using a computer and a variety of software, including Electronic Health Records (EHR) software, Word, Excel, Access, and Outlook. Skill in managing multiple priorities and problem solving. Ability to work effectively under pressure to meet established goals and objectives. Ability to maintain high levels of professional and personal integrity. Ability to convey professional demeanor and sound judgment in dealing with patients' care, protection of patient privacy, and confidentiality. Ability to work independently and in a team environment. Ability to act as a good representative of the company.
EDUCATION & EXPERIENCE
Master of Social Work (MSW) required. A minimum of three (3) years of experience in a healthcare setting utilizing a team approach to patient care required. Provisional Licensed Mental Health Practitioner (PLMHP) or Licensed Mental Health practitioner (LMHP) preferred.
Pay:
$66,390.63 - $72,000.00 per year
