Hospice Social Worker

Well established hospice agency has a need for an experienced Social Worker (MSW) with Hospice or realted experience to see patients in the Merrimack, Nashua, Hampton NH areas. As a member of the interdisciplinary team, the Social Worker reports to the Clinical Manager and is responsible for planning and providing supportive services terminally ill patients and families. Must be comfortable with issues relative to end-of life care, death and dying. Great team in place with a collaborative environment. Full-time, Monday-Friday. No weekends or Holidays required.
