Job Description

Description Lakes Region Community Services is currently accepting applications for a full-time (37.5 hours per week) Service Coordinator to coordinate supports and services for individuals with developmental disabilities or acquired brain injuries and their families. Duties include but are not limited to: facilitate team meetings, write service agreements, provide advocacy, act as a community liaison, develop natural supports, and coordinate referrals/connections to other programs within LRCS. Service Coordinators are responsible for coordinating and communicating with various school districts regarding Special Education Plans, being familiar with In-Home Supports requirements through the Bureau of Developmental Services, and being knowledgeable about family support and transitional services into adulthood, among other things. Support Coordinators are also responsible for assisting in necessary assessments, providing direct support as needed and participating in agency, community and state committees as appropriate. Requirements Knowledge of the Lakes Region, ability to self-manage, excellent communication (both written and oral), time management and organizational skills are required. Qualified candidates will have a Bachelor's degree in a related field, prior experience with developmental disabilities, knowledge of IEPs and 504 Plans, typing and computer skills, a valid driver's license, reliable vehicle and automobile insurance.
