Job Description

Overview The Social Worker at New London Hospital will provide assessment, counseling, information and referral, advocacy services, and other resource assistance to patients in both inpatient and outpatient settings. The Social Worker will act as a resource for the Community Health Workers at both the New London and Newport Primary Care Practices, collaborate with the Inpatient Care Managers to support patient psychosocial needs during hospitalization and as they relate to discharge planning, and respond to referrals from the Emergency Department and other outpatient departments. Responsibilities Provides complete psychosocial assessments of identified patients and families. Provides emotional support to individuals and families facing social and emotional problems. Coordinates community resources to address/meet identified needs. Assists patients through application process for resources, as needed. Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community. Documents objective, concise and pertinent clinical data in the medical record. Acts as a Subject Matter Expert for clinicians and other members of hospital staff. Maintains a listing of emergency resources that can be accessed during non-business hours. Performs other duties as assigned. Qualifications Master's Degree in Social Work (MSW) required Experience in clinical or community care setting preferred Must have excellent organizational skills as well as written and verbal communication skills Must be able to work autonomously as well as in a team environment Demonstrated ability to be flexible and able to function under stressful situations Strong critical-thinking and problem-solving skills Comfortable with basic computer skills (experience with Epic and naviHealth/CarePort a plus) Required Licensure/Certifications
REQUIRED CERTIFICATIONS
• (BLS) Basic Life Support
