Master's degree in social work and New Jersey LSW or LCSW required. This position will primarily support our long-term care residents, helping to enhance their quality of life and ensure needs are met.

Key responsibilities include:
Evaluate existing, new and prospective patients based on their needs, desires and limitations Maintain constant communication with residents while addressing their concerns and goals and helping them keep positive attitudes Coordinate with PT and OT departments to assist in helping residents feel comfortable and confident Ensure compliance with regulatory bodies and in-house clinical guidelines Build rapport with residents/patients and their families and relay all concerns to supervisory case manager Develop care plans for residents and provide therapeutic support as needed Candidate qualifications: Valid license necessary 2-3 years clinical social work experience Valid and current LSW or LCSW license Ability to make psychosocial assessments and develop and implement viable care plans Excellent listening and communication skills In order to provide equal employment and advancement opportunities to all individuals, employment decisions at Greenwood House will be based on merit, qualifications, physical ability to do the work and attendance.
