Social Worker Needed for a Rewarding Role! Exciting Adult Medical Day Care Center is looking for a bilingual Social Worker to create, plan and implement a vital program of Activities that stimulates clients cognitively, culturally, creatively, psychologically, spiritually and physically, and promotes individual and group interaction for pleasure and social interaction. Provide quality assurance reviews at agency's adult medical daycare centers.

Responsibilities:
Pre-admission assessment; psychosocial assessment of participant; assessment of caregiver functioning; preparation of admission forms requiring the signature of the responsible party/caregiver. Facilitate participant and family caregiver pre-admission visit to the Center. Consult with the Administrator to facilitate all admissions and discharges. Ongoing assessment of the social service needs of participants and/or their family caregiver and development of the social service portion of the participant care plan within five days of admission and evaluation of same with progress note at quarterly intervals. Respond to and record of inquiries regarding the service. Development of a schedule which may include: A. Individual, small group and large group activities B. Facility-based and community activities C. Physical exercise or other physical activities D. Intellectual activities E. Social interaction F. Opportunities for both active and passive resident involvement G. Expressive or Empowerment programs Serve as a support and liaison between the participant, family and Center. Counsel to participants as needed. Counsel to family caregivers to assist them in coping with the stresses associated with caregiving. Counsel to staff to assist them in coping with the stresses of caregiving cognitively impaired individuals. Sensory Stimulation programs as needed. Ongoing training on Alzheimer's and other memory impairments. Participate in ongoing process of quality Improvement to evaluate and improve upon services and standards. Provide quarterly quality assurance reviews at the agency's adult medical daycare centers. Maintain and protect the confidentiality of clients' information at all times. Develop and nurture relationships in the community, that will encourage the integration of the clients and the community. Provide tours on an as needed basis. All other duties as assigned.
Skills and Qualifications:
Degree in social work or sociology or psychology with experience in a licensed health care facility (preferably geriatric care facility). Licensed by the New Jersey State Board of Social Work Examiners. Proven customer support experience or experience as a client service representative. Excellent verbal and written communications skills. Strong listening and presentation skills. Ability to respond to common inquiries or complaints from clients, clients' families, regulatory agencies and the public. Ability to define problems, collect data, establish facts and draw valid conclusions. Korean fluency is a must.
Pay:
$25.00 - $28.00 per hour
