Job Description

Social Worker Join our team at Seacrest Rehabilitation and Healthcare Center, a 171-bed Sub-Acute, and Long-Term Care facility where compassion and quality care are at the heart of everything we do. Responsibilities of Social Worker :
Evaluate patients (existing, new and prospective) based on their needs and limitations Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager Build rapport with patients and their families Coordinate care with PT departments Develop care plans for patients
Qualifications of Social Worker:
Must have a CSW or LSW Must be able to work full-time hours and be flexible Bachelor's degree required. Master's degree is strongly preferred. Must be licensed in the state of NJ. Minimum three (1) years' experience as a social worker in a healthcare-related setting. Experience within a nursing home or rehabilitation center is strongly preferred. Knowledge of the rules, regulations, and guidelines that govern nursing care facilities. Communication skills with the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the public. Benefits for
