Nutley Family Service Bureau (NFSB) Inc. is a comprehensive mental health and human service agency that has served Nutley Township and surrounding communities since 1913. Since 2025, NFSB has received accreditation from the Council on Accreditation (COA). We are also licensed by the State of New Jersey to operate and provide Mental Health Outpatient Services. Our accreditation and licensure are a testament to the quality of the services we offer. NFSB provides services in New Jersey, including but not limited to, Essex, Hudson, Bergen, Passaic and neighboring counties.
Position Summary :
We are seeking a compassionate and organized professional to provide person-centered case management and care connection services while facilitating peer support, therapeutic and skill-building groups for individuals living with disabilities and mental health diagnoses. This part-time position (24 hours per week) combines individualized advocacy and resource coordination with group programming designed to promote independence, wellness, and community integration. The ideal candidate is skilled at building trusting relationships, navigating complex service systems, and creating supportive group environments. The Case Management & Group Facilitator position is part of our Inclusive Healthy Communities (IHC) Training, Advocacy and Resource Center which is a community change initiative supported by the Division of Disability Services and aimed at improving, enhancing and effectively increasing community services for people living with disabilities in Nutley Township and surrounding New Jersey communities. Key Responsibilities Case Management & Care Connection Conduct intake assessments to identify each individual's needs, strengths, goals, and barriers to care. Develop, implement, and regularly update person-centered service plans in collaboration with each individual and their support network. Connect individuals to appropriate community resources, including healthcare, behavioral health, housing, benefits, vocational, educational, and social services. Coordinate care across providers, advocating on behalf of individuals to ensure continuity and access to services. Monitor progress toward goals, adjusting plans as needs change and documenting outcomes. Provide crisis intervention and appropriate referrals when needed, following established safety protocols. Maintain accurate, timely, and confidential records in compliance with HIPAA and organizational and funder requirements. Group Facilitation Plan, lead, and evaluate peer support, psychoeducational, skill-building groups on topics such as coping skills, daily living skills, social connection, self-advocacy, and wellness. Create an inclusive, accessible, and welcoming group environment that respects diverse abilities and backgrounds. Adapt group content and delivery to accommodate a range of cognitive, physical, and communication needs. Track attendance, participation, and group outcomes, and document as required. General Collaborate with a multidisciplinary team, attending case conferences and staff meetings. Maintain professional boundaries and adhere to ethical standards and a person-centered, trauma-informed approach. Stay current on community resources, best practices, and relevant regulations. Qualifications Required Bachelor's degree in social work, psychology, human services, counseling, or a related field (or equivalent combination of education and relevant experience). One-two years of experience working with individuals with disabilities and/or mental health diagnoses. Knowledge of community resources and service systems. Strong interpersonal, organizational, and documentation skills. Ability to maintain confidentiality and exercise sound judgment. Preferred
  • Experience facilitating groups in a clinical, rehabilitative, or community setting.
  • Familiarity with person-centered planning, motivational interviewing, and trauma-informed care.
  • Bilingual ability a plus.
Skills & Competencies Empathy and cultural humility; strong written and verbal communication; ability to work both independently and as part of a team; flexibility and problem-solving; comfort working with people in crisis; and proficiency with electronic health records and basic office software.
