Currently seeking a Case Manager- Duties include (but not limited to): To assist youth and their families in identifying, accessing and receiving appropriate mental health and other services in a timely, efficient and effective manner. Participate in the assessment, evaluation and treatment to clients and families under supervision of a licensed social worker. Compile psychosocial history. .Participate in counseling services to client, treatment home parents and natural parents. Provide referral and linkage to ancillary and or specialized treatment services and interventions.
