Job Description

Mint Care Management at The Senior Company is seeking an experienced Licensed Social Worker (LSW/LCSW) to join our geriatric care management team serving older adults and their families throughout New Jersey. This role focuses on providing high-touch, personalized support to aging clients and their families, many of whom live out of state and rely on us as their trusted local advocate. Our care managers help families navigate complex medical, social, and care planning decisions while ensuring clients receive the highest quality care and support. Key Responsibilities
  • Conduct comprehensive in-home assessments
  • Develop and oversee individualized care plans
  • Coordinate and monitor medical care, home care, and community services
  • Accompany clients to medical appointments and advocate when needed
  • Provide ongoing support, guidance, and education to families
  • Assist with care transitions (hospital, rehab, assisted living, memory care)
  • Maintain consistent communication with families and care teams
  • Serve as a trusted liaison between families, providers, and caregivers Qualifications
  • LSW, or LCSW license required (New Jersey)
  • Experience working with older adults, dementia, and complex medical needs
  • Strong knowledge of healthcare systems, long-term care options, and community resources
  • Excellent communication and relationship-building skills
  • Ability to work independently with a high level of professionalism and discretion
  • Reliable transportation required
  • Certification as a Geriatric Care Manager (CCM, CMC, or equivalent) is highly desirable​​​​​​ Why This Role Is Unique Geriatric Care Management offers a level of flexibility and autonomy that is rare in traditional healthcare roles.
Care managers structure their day based on client needs rather than a traditional 9-5 schedule. The work includes a mix of in-home visits, care coordination, family communication, and advocacy, allowing for a dynamic and meaningful workday. Travel is a regular part of this role. Care managers spend a significant amount of time in the field visiting clients, attending appointments, and coordinating care across Northern New Jersey. This role is ideal for a clinician who enjoys working independently, being out in the community, and building long-term relationships with older adults and their families.
